CFTSS Individualized Plan cadence and CANS-NY
How CFTSS IP timing and CANS-NY reassessment interact, and where children’s clinics most commonly slip.
Two clocks running in parallel
Children’s CFTSS providers have two overlapping cadences:
- The Individualized Plan (IP): initial within 30 days, review every 90 days.
- The CANS-NY assessment: at intake, then every 6 months (sooner on significant clinical change).
Why they interact
CANS-NY reassessment findings are supposed to inform the next IP review. When one lands late, the other loses its evidentiary basis. Auditors look for the pair.
Common failure modes
The CANS-NY due date is often tracked in a different spreadsheet than the IP due date. Different owners, different reminders, no cross-check.
How to fix it structurally
